Preventel Health partners with retina and ophthalmology practices to provide ongoing care between office visits. Through Chronic Care Management (CCM), Principal Care Management (PCM), Remote Patient Monitoring (RPM), Behavioral Health Integration (BHI), and Collaborative Care Management (CoCM), our team helps patients with chronic eye conditions stay engaged in their treatment. LPN – Hybrid Clinical & Care Management

About the Role
Preventel Health is seeking a dedicated, compassionate Licensed Practical Nurse (LPN) to join our clinical team in Cleveland, OH. In this role, you’ll combine hands-on clinical care with care management, traveling to facilities across Ohio to provide personalized, high-quality support to patients. This is a client-facing role for someone who enjoys building relationships and takes pride in the details.
Responsibilities
- Communicate effectively with patients, family members, and healthcare professionals to ensure seamless care coordination
- Maintain accurate, detailed records of patient care, treatment plans, and medication administration
- Travel to facilities throughout Ohio to provide care and support
Requirements
- Active LPN license in the state of Ohio
- Minimum of 1 year of experience as an LPN
- At least 1 year of nursing home experience (required; please do not apply without it)
- Willingness to travel to facilities throughout Ohio
- Warm, personable communication style, as this is a concierge, client-facing role
