Supporting the Low Vision Community

At Preventel Health, we bring deep experience and compassion to the low vision community.

Our Expertise

At Preventel Health, we don’t just serve the low vision community — we come from within it. Partner Bari Azman grew up in a professional eye care family, giving him a lifelong understanding of the challenges individuals with vision loss face. With more than a decade of hands-on experience in the field — including 6½ years at a leading Mid-Atlantic low vision practice — Bari has worked directly with thousands of patients, families, and providers.

During this time, he guided patients through the emotional and practical realities of sight loss, supported families in adapting to changing needs, built international community support groups, collaborated with assistive technology startups to test and advance innovative solutions, and partnered with retina specialists and ophthalmologists to expand access to comprehensive care. These experiences gave him unique insight into the real barriers patients face every day — from mobility and independence challenges to the financial burden of out-of-pocket low vision services.

This understanding drives Preventel’s mission to make quality care more accessible. With our preventative care model, supported by Medicare-approved billable services, Preventel delivers the extra layer of support that low vision patients deserve — without adding financial strain. By combining family legacy, professional expertise, and innovative care models, Preventel is uniquely positioned to break down barriers, improve patient independence and quality of life, and help practices deliver compassionate, sustainable care.

This is why Preventel Health is trusted to bring not just clinical services, but genuine understanding, to the low vision community.

Our Mission

Providing greater independence and improving quality of life for patients with low vision is at the heart of our mission. We are committed to breaking down the barriers that stand in the way of care, ensuring every patient has access to the support and resources they deserve.

An illustrated scene of a busy train station platform where several blind and low vision individuals use white canes to navigate. A man in a yellow jacket and blue hat walks with his cane while a woman with curly hair walks beside him. Another man in an orange coat smiles while holding a white cane and a smartphone. A woman in a red coat also uses a cane while checking her phone. Other commuters with luggage and backpacks move around them. Our Solution

Preventel Health bridges the gap between patient needs and practice resources by bringing a proven preventative care model to the low vision community. Traditionally, many patients have been left without consistent follow-up and support because low vision services are often considered an out-of-pocket expense. This creates unnecessary barriers at a time when patients most need guidance, encouragement, and medical oversight. Preventel changes that reality by delivering care through Medicare-approved, billable services — ensuring patients receive essential support without the added financial strain.

Our model integrates seamlessly into retina and ophthalmology practices, enhancing the care patients already receive. We take on the responsibility of ongoing patient engagement, education, and support, so providers can remain focused on delivering specialized retinal treatment. Through regular check-ins, proactive care management, and coordination with families and caregivers, we address the daily challenges low vision patients face — from medication adherence and lifestyle adjustments to emotional support and community resources.

By combining clinical expertise with deep experience in low vision care, Preventel provides a new dimension of support that strengthens patient independence and quality of life. At the same time, our services help practices reduce administrative burdens, improve patient satisfaction, and align with evolving Medicare reimbursement opportunities. In short, our solution not only empowers patients, but also sustains practices, creating a system where everyone benefits.

Together, we can make a lasting difference in the lives of low vision patients.
